STREAMLINED MEDICAL RECORD REQUESTS FOR LAW FIRMS

Log in to your QuestLynk client portal and complete the Medical Record Request Form. In this form, you will provide your organization’s information, your client’s information, the facility’s information, the type of records requested, the signed Protected Health Information authorization form, and a copy of your client’s identification, if required.

You will receive weekly status updates at the email address connected to your QuestLynk client portal. Updates will also be posted in the portal under your client’s task.
Please monitor your email and portal for the following important updates:
Confirmation that your request has been received by the facility.
Confirmation that an invoice has been issued by the facility, if applicable.
Confirmation that the records have been dispatched by the facility.

Once the facility confirms that the records have been sent, and your firm confirms that the records have been received, QuestLynk will update the request status to Complete.
To communicate with QuestLynk regarding a specific record request, open that client’s task in the portal and leave a comment.
